Real World Ready

The Health Transition Alliance is a partnership between North Oaks Health System and SLU's College of Nursing and Health Sciences. The Alliance consists of an interdisciplinary team of medical staff that includes registered nurses, a social worker, a physician, a full-time health coach, along with a group of student health coaches who are senior interns. The HTA program staff and students work with patients in their home to assist them with disease self-management, symptom control, communication and follow-up with providers, as well as providing nutritional education, medication education, and the identification of community resources. The primary objective of the HTA program is to reduce the risk factors and patient behaviors that contribute to an exacerbation of the patient’s medical conditions that may lead to a hospital readmission.
