This form is to be provided to a licensed mental health provider by the student and
returned to our office. Discharge must be completed in order to meet the sanction
requirement.
Student may present this form to the mental health professional administering the
SASSI assessment to show successfully completion and recommendations.
Students are provided a menu of educational assignments to choose from, allowing them
to participate in developing their own individualized learning plan.
The purpose of this assignment is to engage the student in purposeful thought about decision-making,
decision traps, and habits and identify how to make positive choices.
