Sending Referrals

All service referrals along with any questions about services are to be sent to [email protected]. Files must be unzipped. 

Parenting and Family Skills Building Referrals – Any agency or Self referrals are welcomed

Include one of the following documents. The information is used to facilitate the intervention around the reason family is involved with DCFS. 

  1. CPI Investigation Report – Form 10
  2. Case plan with section detailing the specifics of why family is involved with DCFS.
  3. Your assessment of the family 
  4. Court papers that detail the specifics of what happened in order for family to be involved with DCFS.
