All immunization records and/or Proof of Immunization Compliance Forms can be submitted to the University Health Center by emailing them to [email protected].
Tetanus is only good for 10 years. If you have health insurance, you can walk into most any pharmacy and get a tetanus shot for free with your insurance card. We suggest Walmart because they put your vaccine into your immunization record in the state system; others don’t always put them in the system. If you do not have health insurance, you can go to your local parish health unit and get a vaccine. The Hammond Health Unit on Club Deluxe Road does vaccines for $10 by appointment only. Two (2) doses given any time after the age of 1. The second dose should not have been given within 30 days of the first dose. Titers will be accepted for this requirement.
Two (2) doses of meningococcal conjugate vaccination separated by at least eight weeks. If the first dose is after the age of sixteen, only one dose is required.
