Below is a list of various health insurances provided to Southeastern Louisiana University
Employees. New employees must enroll in a group benefit plan within the first 30 days of employment. If this deadline is missed, the employee can enroll in health insurance during Annual
Enrollment or if they have an IRS-approved qualifying event.

IMPORTANT: A new enrollment form must be completed for each new dependent within 30 days after
acquiring the new dependent (birth, adoption, marriage, etc.). Failure to comply with
this rule, may subject your dependent to the possibility of being ineligible for health
coverage until the next plan year.

Employees who have 10 or more work days of leave without pay in a calendar month will
be responsible for paying the employee and the employer portions of their health insurance
premiums.

NOTIFICATION OF CHANGE ERROR: It is the employee’s responsibility to notify the program of any change or error
in classification of coverage or any other error affecting his contribution amount.
Any failure later determined will be corrected on the first of the following month.
All refunds shall be limited to six months from the date notice is received by State
Employees Group Benefits.
