Select Dental II

Select Dental II insurance is available to eligible new employees to enroll within
30 days of their hire date. Other eligible employees may enroll at anytime during
the year. The plan is administered by American Public Life Insurance Company. There
are three levels of coverage, Option A, Option B, and Option C. The higher the option,
the more the plan pays toward covered services and the higher the premium. Plan Highlights include:

• Your choice of dentist
• No deductible on Preventive and Adjunctive
• No waiting period for Preventive and Diagnostic Services
• 3 month waiting period for Basic Restorative
• 9 month waiting period for Major Restorative, Endodontics, Periodontics, Prosthetics-Removable,
  Fixed Bridge, Oral Surgery and Adjunctive Services.
• 24 month waiting period for Orthodontic benefits for children
• $50.00 deductible all services except Preventive and Adjunctive
• Annual maximum of $1,000
• Benefits can be coordinated with Careington network for a greater savings

Benefits will be paid only for specific ADA codes as listed in the policy when a charge
is incurred for the covered dental treatment while coverage is in force.
