Plan 1 Employee Only
Faculty: $20.70 - monthly
Staff: $7.97 - bi-weekly

 

Plan 2 Employee + 1 Family Member
Faculty: $37.30 - monthly
Staff: $14.35 - bi-weekly

 

Plan 3 Employee + 2 Family Members
Faculty: $45.50 - monthly
Staff: $17.50 - bi-weekly

 

Plan 4 Employee + 3-6 Family Members
Faculty: $53.80 - monthly
Staff: $20.70 -  i-weekly

 

Faculty payroll deduction amounts reflect a 10-month membership. Staff payroll deduction amounts reflect a 12-month membership.
